Accident Investigation

First, it is important to understand what an accident is.

​

An accident is an unintentional, undesirable, and unplanned event that could have been prevented had circumstances leading up to the accident been identified, and acted upon, prior to its occurrence.

​

Nobody wants an accident or ill health to befall anyone, let alone to employees, customers or visitors in your own workplace.

​

It is a sad fact of life, that such occurrences do still occur, and it is important to fully investigate all accidents and incidents (including near misses) primarily to determine what went wrong and what needs to be done to prevent any recurrence.

​

According to the Labour Force Survey, over 40 million working days are lost through work-related injuries and ill health, at a cost to business of 2.5 billion pounds.

​

Insurance companies are requiring more and more information regarding any accident or incident which results in the company (or injured party) claiming on their policy.

​

In this day and age, there is quite a claims culture with many advertisements stating “where there’s blame there’s a claim”, which can lead to dishonest, fraudulent claims by individuals or indeed groups of people purporting an accident has taken place.

​

An effective investigation requires a methodical, structured approach to information gathering, collation and analysis.

​

We, unfortunately, have been involved in many accident investigations, sadly some resulted in fatalities.

​

Some of the investigations we have conducted identified malicious acts and fraudulent events resulting in individuals being dismissed from the company for gross misconduct.  In one case an individual was also pursued by the Health and Safety Executive under Section 7 of the HSWA for failing to take reasonable care for the health and safety of themselves and of other persons who may be affected by their acts and omissions at work.
